covid screening form

Please complete the COVID-19 screening form within 24 hours of attending your appointment. If you have cold/flu or other symptoms related to Covid-19 please get in touch to cancel or reschedule as soon as possible.

SAFETY MEASURES

Please arrive to the clinic wearing your face mask.
I have put some guidelines and safety measures together to better help prevent infection in the clinic. It is important that you read the guidelines prior to your appointment

CANCELLATION & policies

If you are a new client:

Your consultation form needs to be filled out upon booking. Please fill in the following consultation form before attending your appointment. This allows me to better prepare your treatment session as well as reducing contact within the clinic.

If you are unsure whether any of the treatments I provide is for you, I am open to a phone consultation to provide advice on what may be best for you at this moment.

To minimise contact please bring the exact amount and place it in an envelope that I will provide.

Card payments:

All card payments are processed online and in advance of your treatment.

Covid Screening Form

Please fill in the covid screening form within 24 hours in advance of your treatment.
If you have a cold/flu symptoms, have been in close contact with a positive covid case, or have been abroad 14 days before your scheduled appointment please cancel or reschedule your appointment as soon as possible. Read about cancellation policy.

Please arrive in time for your appointment. Do not arrive too early, do not come in late. There will be no extra time added at the end of your treatment if you arrive late to your appointment.

Please do not bring anyone with you the day of your treatment. No one other than clients who are to receive treatments are allowed to be in the waiting room.

The use of a mask inside the clinic is mandatory. Please wear your mask adequately upon entrance and follow the cough and sneez etiquette.

Feel free to bring your own water, blanket, eye pillow if this makes your treatment more comfortable.

Sanitize/ Wash your hands thoroughly before entering the treatment room and avoid touching surfaces. I will provide a  box for you to place your items.

I have added extra measures and extra time between appointments to allow time to clean and disinfect all areas thoroughly. As always every client will receive fresh clean sheets and towels.

Cancellation policy

 You may reschedule or cancel your appointment up to 24 hours in advance of your appointment time. If less than 24hours is given you will be liable for 100% of the treatment fee you booked

Please understand that the clinic has been closed for a long period of time and therefore there are clients waiting to be treated. Giving less than 24 hours notice isn't sufficient for me to fill in the slot you booked and may incur clients in who are on the waiting list to lose the chance to receive the treatment they may need.

No shows

Clients who either forget or consciously choose not to avail of their appointment for whatever reason will be considered a “no-show”.  
Clients who don't show the day of their appointment will receive an invoice/ payment link for the full amount of the treatment fee they missed, and this missed fee will be required in order to rebook another appointment.
Non-shows are considered a serious lack of respect, not only for the therapist but also for clients who may be waiting to receive treatment. 
Therefore, I reserve the right to decline future appointments or ask for the owed fee upon the next booking when a non-show has been experienced. I reserve the right to cancel or decline bookings where a boundary between client and therapist has been trespassed by the client.

 

Arriving in time

Appointment times have been arranged specifically for you.
Clients must arrive in time for their appointment. Please do not arrive more than 5 minutes early and for your own benefit do not arrive late.

Clients who show up late will not be given extra time. If you are late for your appointment the actual time of your treatment may be shortened to accommodate other appointments that follow yours.
Time must be respected both ways. If you are considerably late for your appointment and decide to cancel your appointment, your fee will be due and non-refundable
